* FOSTER: was in ACC foster care
* OWNER SUR: Surrendered to shelter by owner
* RETURNED: Returned adoption
* SEIZED: removed from home due to eviction/arrest
* STRAY: surrendered to shelter as a stray.

* ABANDON: animal was abandoned
* ALLERGIES: household member allergic
* ATT ANIMAL: person surrendering claims animal bit/scratched other animal
* ATT PEOPLE: person surrendering claims animal bit or scratched human
* BORNSHELTR: animal was born at shelter
* DESTRUCTIV: animal destructive
* FOSTER END: ACC foster returned to shelter
* HOARDING: too many pets
* HOUSE SOIL: animal not housebroken/litter box trained
* INADFACIL: owner could no longer supply housing for animal
* LLORDPRIVA: animal not allowed in home per landlord
* MOVE2PRIVA: owner moving to housing that pets are not welcome
* NEW BABY: owner had a baby
* NO TIME: owner does not have time to care for pet
* OWN ARREST: owner was arrested
* OWN EVICT: owner evicted from home
* OWN HOSP: owner is in hospital
* OWNER SICK: owner is sick and unable to care for animal
* NO ANSWER: no reason for surrender given
* PERS PROB: owner is having personal problems which prevent them from caring for animal
* PETS CONFL: The animal does not get along with other animals in the home.
* PET HEALTH: owner claims animal is in poor health
* SEARCH WAR: seized by police
* TOO MANY P: owner felt they had too many pets

Reason for New Hope (reason animal is on list)

* DISEAS-ILL: animal is ill (ranges from simple upper resp to sever conditions)
* INJURED: animal is injured
* SUCKLING: animal too young to eat on own
* TEMPER: animal has been rated poorly during evaluation (ACC behavior eval is questionable… read eval to determine)
* TOOYOUNG: animal is under 8 weeks old

Placement Status Ratings

N – NONE – No behavior rating
B – BEGINNER – Beginner home
A – AVERAGE – Average home
E – EXPERIENCE – Experienced home with specific counseling
EA – EXPNOCHILD – Experienced home, no children, with specific counseling.
H – NH ONLY – New Hope partner placement only
NP – Not available for placement

Medical Ratings

“The status system is on a scale of 1 to 5, with designations C, NC, and G:”

* C = contagious
* NC = non-contagious
* G = pregnant

* 1–NORMAL CONDITIONS: animal is healthy
* 2–MINOR CONDITIONS: Cat has a condition that requires limited or no treatment such as mild dental tartar (2NC), obesity (2NC), or fleas (2C).”
* 3–MAJOR CONDITIONS: animal has a condition that will require medical but is treatable
* 4–SEVERE CONDITIONS:animal has a major illness/injury
* 5–VERY SEVERE CONDITIONS: animal has a severe and potentially life threatening condition
